Facility Rental Agreement

Thank you for your interest in using the Young Israel of Hewlett for a simcha, rental or special Event. Please fill in the information below, download the attached form and release information and then click Submit and Pay. You can send the completed, signed form to office@yihewlett.org

Please Select OneRoom Rental - Member Fee -$250Room Rental - Non-Member Fee - $500Food and Beverages must remain downstairs in the social hall.

Setup/Cleanup

Enter the number of hours needed for Setup and/or Cleanup. $20/hr.
Entering Zero or nothing means you are responsible for setting up and cleaning up. You will be billed a cleaning fee at $25/hr if additional cleanup is required after your event.

Security for Front Door

Enter the number of hours needed for Security to man the front door. $20/hour Entering zero means you are taking responsibility for having a person at the front door at all times. The door cannot be propped open.

I agreeI do not AgreeBy checking above you agree to this rental agreement (the “Rental Agreement”). The undersigned (the “Licensee”) has an exclusive license to use the premises of the Young Israel of Hewlett (“YIH”) as indicated in the Rental Agreement above (the “Premises”).
Licensee agrees to indemnify and hold YIH and its premises, agents, personnel and affiliates harmless and blameless for any loss, damages, claims, demands, or judgments including, without limitation, claims for personal injury or wrongful death resulting from Licensee’s use of the Premises (the “Losses”) other than Losses caused by YIH’s gross negligence or intentional misconduct. Licensee agrees that it is responsible to compensate YIH for any Losses caused by Licensee, its staff, or its invitees.
